Collective Health Earns Renewal of its Two-Year NCQA Case Management Accreditation for Care Navigation™ Program

SEPTEMBER 18, 2024, San Francisco, Calif. Collective Health, the leading employee health benefits platform with integrated member advocacy, care navigation, and an extensive partner ecosystem, today announced it has received its two-year Case Management Accreditation from the National Committee for Quality Assurance (NCQA) for its Care Navigation™ program.

The accreditation recognizes that Collective Health has yet again met NCQA’s rigorous national standards, developed with input from experts across the field, in key areas like identification, assessment, care planning, monitoring, and coordination. By achieving this for the second time, Collective Health’s Care Navigation™ program demonstrates its ability to consistently deliver efficient and effective case management services.

“Our approach to Care Navigation™ is built on the idea of supporting members at every stage of their healthcare journey, while providing them with the knowledge and resources that will have a lasting impact,” said Ben Becker, DNP, RN, Senior Director of Care Navigation at Collective Health. “This recognition affirms the hard work we’ve continuously put into growing a care management and navigation program that adheres to the highest quality standards. Our team is wholeheartedly committed to guiding people through their benefits and care options, helping them achieve their best health outcomes.”

Collective Health was one of the few TPA complex case management programs to receive accreditation for meeting these rigorous service and quality standards back in 2022. This recertification demonstrates Collective Health’s Care Navigation program’s commitment to using evidence-based standards, identifying opportunities for improvement, and raising operational efficiencies.

“Case Management Accreditation moves us closer to measuring quality across population health management initiatives,” said Margaret E. O’Kane, President, NCQA. “Not only does it add value to existing quality improvement efforts; it also demonstrates an organization’s commitment to the highest degree of improving the quality of their patients’ care.”

Fragmented care can lead to higher costs and a frustrating experience for members. Collective Health’s Care Navigation™ offers holistic care management and navigation through a dedicated team of pharmacists, social workers, nurses, dietitians, and care coordinators. This team supports members with chronic or severe illnesses, guiding them through the complex U.S. healthcare system. By leveraging its role as an employer’s plan administrator and accessing real-time claims and engagement data, Collective Health quickly identifies members needing support, helping them make informed healthcare choices that improve care and reduce costs. By using Collective Health’s Care Navigation™ program, employers can save an average of $7,100 savings per engaged member.
