Highest-Level Three-Year NCQA Accreditation Marks Third Consecutive Accreditation for Care Navigation
San Francisco, California – September 30, 2026 – Collective Health, a leading independent third-party administrator (TPA) and Care Navigator uniting administration, cost management, and care, today announced that its Care Navigation program has earned a three-year Case Management Accreditation — the highest level awarded — from the National Committee for Quality Assurance (NCQA), marking the program’s third consecutive NCQA accreditation. Care Navigation is a proactive clinical layer that guides members through their care journeys, including complex care, while de-risking rising-cost claimants for employers. The distinction follows a comprehensive review and audit process and reinforces the strength of Collective Health’s approach to care management.
Collective Health’s Care Navigation program guides members toward higher-value care before costs escalate. The program has a 91% member satisfaction rate¹. That impact comes from an interdisciplinary team of pharmacists, registered nurses, social workers, registered dietitians, and care coordinators, all working within Collective Health’s connected platform — Collective TPA™, Care Navigation, and My Collective® — to guide members through every step of their care journey.
“This is our third consecutive NCQA accreditation for Care Navigation, and the first time we’ve reached the three-year term — the highest level NCQA awards. It shows we’re not just maintaining a standard, but continuing to raise it for the populations we serve,” said Ari Hoffman, SVP of Product and Chief Clinical Officer at Collective Health. “Navigating a new diagnosis — whether it’s cancer, a behavioral health condition, or a chronic illness — is hard enough without also trying to figure out the system. Care Navigation exists to close that gap: our clinical team steps in early, before a condition becomes a crisis, so members get the case management support they need to stay on track, get healthier outcomes, and afford the care they need.”
“Case Management Accreditation moves us closer to measuring quality across population health management initiatives,” said Margaret E. O’Kane, President, NCQA. “Not only does it add value to existing quality improvement efforts; it also demonstrates an organization’s commitment to the highest degree of improving the quality of their patients’ care.”
NCQA Case Management Accreditation is a nationally recognized evaluation that provides a framework for implementing evidence-based best practices and ensuring efficient, cost-effective case management processes and service delivery. NCQA developed its accreditation standards with input from researchers, employers, regulators, and other field experts.
